What happens when you are referred to GIM Clinic.

STEP 1 - Greeting Phone Call

Once the referral is received, we will contact you to complete your information and send our intake questionnaires and forms to be filled.

STEP 2 - Review Information

We will review your medical history to determine if any specific blood work or other tests should be completed before the appointment. We will also obtain an updated medication list, if needed. Once the questionnaires are complete and all the test results are received, we will schedule an assessment.

STEP 3 - Assessment

Our physicians will review the medical history, tests completed and medication list. Also perform a physical examination and provide any specific instructions. That information will be sent to your referring doctor.

We will answer any questions you may have and ensure you are well-informed about what to do next.
