Obesity Medicine

Obesity is not a choice. And it is not a personal failure.

If you’ve tried hard and still feel stuck, you’re not alone.

Obesity is a complex medical condition — and effective, evidence‑based care exists to support your health over time.

A medical approach to obesity – focused on long‑term health

You don’t have to do this alone — we’re here to guide you, step by step.

At GIM Clinic, we focus on obesity and lifestyle medicine through the Guided Obesity and Lifestyle Program™ (GOALS Program™): a medical approach that treats obesity as the complex, chronic disease it truly is.

Obesity isn’t just about weight — it can affect heart health, metabolism, joints, and mental health. Our goal is to help you make safe, sustainable progress while improving overall health and quality of life.

Many patients tell us that having structured follow‑up from a compassionate and knowledgeable physician makes all the difference.

OHIP-funded obesity medicine care

Your physician care is fully covered by OHIP — so you can focus on your health, not the cost.

This means medical assessments, follow‑up, and obesity medicine care are fully covered with a referral. In other words, there are no private fees for physician care.

How to get started?

  1. Ask for a referral: ask your family doctor, nurse practitioner, or another clinician you see to refer you for OHIP-covered obesity medicine care. You can simply say: “I’d like a referral for the OHIP‑covered obesity medicine care at GIM Clinic.”
  2. Send the referral: the practitioner can fax the referral or send an electronic referral via Ocean.
  3. We take it from here.

You can download the form here.

Or ask your doctor to refer you online.

Why Refer to the GOALS™ Program

Supporting your patients with obesity and chronic conditions—within the OHIP framework.

Managing obesity, diabetes, hypertension, and other chronic conditions often requires more than brief office visits. Patients need consistent support, accountability, and education to make meaningful progress. The GOALS Program (OHIP pathway) provides exactly that—without additional cost to patients.

What Patients Receive

  • Weekly electronic follow‑up: Patients are contacted regularly through secure online questionnaires, keeping them engaged and accountable between visits.
  • Evidence‑based lifestyle guidance: Practical tools and education help patients make changes that improve metabolic health.
  • Continuity of care: All progress is communicated back to you, so you remain central to the patient’s journey.

How This Benefits Your Practice

  • Better outcomes: Patients return with improved weight, blood pressure, and metabolic markers—making your management easier.
  • Reduced burden: Weekly electronic follow‑up means patients receive ongoing support outside your office, without adding to your workload.
  • Enhanced patient satisfaction: Patients appreciate proactive, structured care that goes beyond medication adjustments.
  • Collaborative partnership: GOALS complements your practice by reinforcing your recommendations and extending your reach.

Refer today. With GOALS, your patients gain consistent support, and you gain a trusted partner in chronic disease management.
