Acute Internal Medicine Service (AIMS)

Streamlined Access to Internal Medicine Management

Welcome to AIMS at Guelph Internal Medicine Clinic®

Our Acute Internal Medicine Service (AIMS) provides rapid assessment, investigation, and initial management for adult patients with new or worsening medical conditions. We help stabilize undifferentiated or urgent presentations, facilitate necessary workup, and arrange a safe transition back to the Primary Care Provider (PCP) or referring specialist.


Please note: Our team specializes in undifferentiated or urgent presentations, as well as acute decompensations of chronic illnesses. Routine management of stable chronic diseases remains with the patient’s PCP or specialist.


How Does AIMS Work?

Most patients are seen for a series of 3–5 visits, allowing for thorough assessment, treatment initiation, and stabilization. Once appropriate, patients are transitioned back to their PCP or referring provider for ongoing care.

Who Should Be Referred?

Consider referring patients with:

  • New/uncontrolled HTN
  • New oxygen desaturation
  • AKI or new CKD
  • New or decompensated CHF
  • New or decompensated COPD
  • Electrolyte disturbances
  • Chest pain, not yet diagnosed
  • New or uncontrolled diabetes
  • New hepatitis or cirrhosis
  • New shortness of breath NYD
  • New hypo/hyperthyroidism
  • New diarrhea or colitis, NYD
  • New peripheral neuropathy
  • Anemia/ thrombocytopenia
  • Unintentional weight loss
  • Initial malignancy workup
  • Cellulitis or other infections
  • Fever of unknown origin
  • Post-discharge evaluation
